1. Hazard Identification
• Hazard: Risk of trips, slips, or falls when using the stairs and single steps within the premises.
• Location of Hazard:
• Entrance: Four steps leading to the lower landing inside the main door.
• Main Staircase: Single-story staircase from the lower landing to the first floor where the clinic is located.
• Within the Clinic: A single step leading from the waiting area into the clinical procedure room.
• Who is at Risk?
• Patients, including elderly and mobility-impaired individuals.
• Clinic staff.
• Visitors, including delivery personnel.

• Entrance Steps
• Steps are regularly inspected to ensure they remain in good condition.
• Slip-resistant strips applied where necessary to reduce the risk of slipping.
• Handrails installed on both sides for patient and visitor support.
• Main Staircase
• PIR (Passive Infrared) lighting installed to activate upon motion, ensuring the stairs are always well-lit.
• Bannisters/grab rails are securely fixed on both sides of the staircase for added support.
• Regular cleaning and maintenance to prevent accumulation of dust or spillages that may increase risk.
• Single Step from Waiting Area to Procedure Room
• High-visibility edging applied to the step to highlight the change in floor level.
• Clear signage is installed to warn people about the step.
• Staff trained to remind and assist patients who may have difficulty navigating the step.
4. Additional Safety Measures

• Patients are informed upon arrival of any steps they may need to navigate.
• Patients with mobility concerns are offered assistance.

• Staff trained in first aid to respond in the event of a fall.
